Waiver and Release of Liability
_The undersigned attests:
Acknowledges and Represents that participant is in good physical condition and that participant has no pre-existing condition, disability, illness, or other condition that could prevent participant from participating in Atlanta Calcio Academy training activities without risk of serious injury or impairment of health.
I/We, the above Participant(s) and/or spouse and/or parents/guardians of the above Participant understand and acknowledge that Atlanta Calcio Academy is an independent legally licensed entity operating as RGE, LLC. And that any reference to Atlanta Calcio Academy includes RGE, LLC; Gavino Asteghene and officers of RGE, LLC, All the coaching staff of Empoli FC, Empoli FC Academy USA, Atlanta Calcio Academy, Atlanta Academy USA, Empoli FC Giovani.
Acknowledges and Understands that each participant will be engaging in activities that involve risk of serious injury, including disability, illness, or death, and severe social and economic losses which might result not only from their own actions, inactions or negligence, but the action, inaction, or negligence of others, the rules of play, or the conditional of the premises or of any equipment used. Further, that there may be other risks not known to us or not reasonably foreseeable at this time;
Assumes all the Foregoing risk and accept personal responsibility for the damages following such injury, disability, illness or death;
Releases, Waives, Discharges and Covenants Not to Sue Empoli FC Academy USA, Empoli FC Giovani, RGE, LLC., Atlanta Calcio Academy, Atlanta Academy USA, Gavino Asteghene, Their respective agents, directors, coaches, and other employees of the organization, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and leasers of the premises used to conduct the event, all of which are hereinafter referred to as “Releasees,” from any and all liability to each of the undersigned, his or her heirs and next of kin for any and ALL claims, demands, losses, or damages on account of injury, including by but not limited to death or damage to property, caused or alleged to be caused in whole or in part by the negligence of the Releasees or otherwise.
I/We understand that no refunds will be granted after camp has begun. NO exceptions.
I hereby give Atlanta Calcio Academy, Atlanta Academy USA ,Empoli FC Academy USA ,RGE,LLC permission to take photographs of participant or photographs in which participant may be involved with others without compensation to participant or me. These photographs may be used by the above Academies for promotional and information purposes in print, on the Atlanta Calcio Academy, Atlanta Academy USA ,Empoli FC Academy USA website and in other media.
